Ladder Safety in Canada: Hold the Rungs, Not the Rails (2026)

Almost everyone climbs with their hands on the rails. It feels natural, it keeps your hands clear of the rungs your feet are using, and it is the grip that fails at the exact moment you need it. A rail gives you friction. A rung gives you something to hang from.
Why ladder falls happen
CCOHS lists four recurring causes: using the wrong ladder for the job, using ladders that are defective or in poor condition, improper care or use including incorrect positioning and poor footing, and workers not being trained adequately.
Notice what is missing. None of those is bad luck or a freak event. Falls from portable ladders are a major source of serious injury, and the causes are the same handful of decisions repeated across thousands of sites.
The two mechanical failures that dominate are the base sliding out and the ladder tipping sideways. Both are set-up failures rather than climbing failures, which means most of the risk is decided before anyone puts a foot on a rung.
Overreaching is the third. The rule of thumb is to keep your belt buckle between the side rails. The moment it passes outside them, your weight is working to tip the ladder rather than hold it down.
Set-up: the 4:1 rule and what sits around it
For every four units of height to the support point, the base goes one unit out from the wall. That produces roughly a 75 degree angle, which is the geometry that stops the base sliding.
There is a quick field check for it. Stand with your toes against the feet of the ladder and put your arms straight out at shoulder height. If your fingertips touch the rung nearest shoulder level, the angle is about right.
Where a ladder is used for access rather than as a work position, it needs to rise about one metre, or three feet, above the landing so there is something to hold while stepping on and off. On an extension ladder, do not climb above the fourth rung from the top.
Set up on solid, dry, level ground clear of ice, snow, water and sand. Secure the ladder at the top, and at the bottom too where it can be done. A helper footing the ladder is a control, not a courtesy.
Keep the base area clear, and lock or otherwise secure any door that opens into the ladder. A door swinging into a ladder is a recurring incident cause that costs nothing to prevent.
Canadian grades are not American types
Duty rating tells you how much the ladder is built to carry, including the worker, tools, and anything being carried up. It is printed on the side rail, which is where to look rather than guessing from the ladder's weight or price.
Canada uses a grade system under CSA Z11-18, reaffirmed in 2022: grade 3 is light duty, grade 2 medium duty, grade 1 heavy duty, with grade 1A and 1AA above that. Roughly, grade 3 carries around 200 lb, grade 2 around 225 lb, grade 1 around 250 lb and grade 1A around 300 lb.
Most guidance you will find online uses the American Type I, II and III system instead. The concepts map loosely and the markings do not, so read the rail rather than translating.
Construction, industrial and utility work generally calls for heavy duty or better. A household grade 3 ladder on a job site is not a budget decision, it is an overloaded ladder.
Add it up honestly: your weight, your boots and outerwear, a tool belt, and whatever you are carrying. Crews routinely exceed a grade 2 rating without noticing.

Footwear, and a nuance worth knowing
CCOHS is specific about this: wear protective footwear with slip-resistant soles and heels. Both halves matter, and the heel is the part people overlook.
On a rung, a defined heel catches the back edge and stops the foot sliding forward off it. A completely flat sole has nothing to catch with, so it relies entirely on friction against a narrow round bar, often wet.
That is worth flagging because the right geometry depends on the surface. Flat soles give better contact on a narrow beam flange, which is why structural work favours them. On ladder rungs, a heel earns its place. A crew that does both will be compromising somewhere, and the honest answer is to know which surface dominates your day.

Three points of contact, and one person
Maintain three points of contact at all times: two hands and one foot, or two feet and one hand. That is the rule most workers can recite and fewest follow, because carrying anything breaks it immediately.
Which is the actual instruction hiding inside it. If you need both hands to carry something, you cannot climb safely, so the material goes up another way: a hoist line, a tool belt, a bag, or a second trip.
Face the ladder going up and coming down, every time. Turning to descend is a common and entirely avoidable cause of falls.
Only one person on a single-width ladder, and only one person per side on a double-width one. Two people on a ladder built for one exceeds the duty rating and changes how it behaves.
Inspect before every use. Look for loose or broken rungs, steps, rails and bracing, and for loose screws, bolts and hinges. A damaged ladder comes out of service immediately rather than being flagged for later, and a single hairline crack in a rail is enough.
Electrical work changes the ladder
Metal and wet or contaminated ladders conduct. Keep them away from electrical parts and components, and use a non-conductive ladder for any work near energized equipment or overhead lines.
Fibreglass is the usual answer for electrical work, and it needs to be kept clean and dry, because contamination on the rails can provide a conductive path the material itself would not.
Outdoors, overhead lines are the hazard that catches people during set-up rather than during the work. A ladder being carried upright or raised near a service drop has a long reach, and contact does not require touching.

Sometimes the ladder is the wrong tool
This is the part that sits above every rule in the article.
Quebec's prevention guidance puts it plainly: the best way to prevent falls is to work on the ground whenever possible. Ontario construction guidance follows the same logic, directing that where conditions allow the use of a scaffold, elevated work platform or other equipment, that should be used instead of a ladder.
That is the hierarchy of controls applied to access. A ladder is a personal, unstable, one-person work position, and a scaffold or platform removes the hazard rather than managing it. The reason ladders win so often is speed of setup, which is the same reasoning that appears in fall investigations.
There is also a threshold worth knowing. Ontario guidance notes that where a worker on a ladder is at a location from which they could fall more than three metres, they must be adequately protected from the falling hazard using fall protection. Many workers assume a ladder means no harness is needed.

Storage and transport
Ladders get damaged between jobs more often than during them, and damage found on site is damage that travelled there.
Secure ladders when transporting them by vehicle so movement and vibration cannot work them loose or flex them. A ladder bouncing on a rack for a hundred kilometres is being fatigued.
Store stepladders closed and vertical. Store single and extension ladders horizontally in racks with enough support points to prevent bending or distortion. A ladder stored sagging between two points will not be straight when you need it.
Keep them clean of mud, chemicals and debris, both because contamination degrades the material and because it hides the defects an inspection is meant to find.
Inspect ladders that have been unused for a while before putting them back into service. Storage is not a neutral state, and a ladder that has sat through a winter in a damp shed deserves a proper look.
